RP可能在婴儿期开始,最初的症状通常出现在成年早期,从夜盲症开始。随着疾病的进展,患者视力逐渐丧失。在后期阶段,RP可破坏黄斑中的光感受器(其负责良好的中央视力)。据估计,RP影响美国至少7.5万人和全世界150万人。
 
已知有超过60种不同的基因与RP相关,因此很难开发出该疾病的模型。动物模型虽然有用,但是由于种间差异,有很大的局限性。研究人员还利用来自眼睛库的人视网膜细胞来研究RP。虽然这些细胞反映了疾病的最后阶段,但是,它们很少反映出疾病是如何发展的。因为从患者获取视网膜细胞非常危险,所以目前还没有RP的人组织培养模型。最后,人类胚胎干细胞在RP研究中可能是有用的,但是它们充满了道德、法律和技术问题。
 
iPS技术的使用为这些限制和问题提供了一种途径。研究人员可以诱导患者自身的皮肤细胞恢复到一个更基本的、胚胎干细胞样的状态。这种细胞是“多能性”的,意味着它们可以被转化为各种类型的特化细胞。
 
在目前的研究中,CUMC研究小组使用iPS技术,将取自两位RP患者(每名患者具有一个不同的MFRP突变)的皮肤细胞,转化为视网膜细胞,从而制备了患者特异性模型用于疾病研究和检测潜在的治疗方法。
 
通过分析这些细胞,研究人员发现,MFRP的基因突变的主要作用是,破坏肌动蛋白的调节,肌动蛋白是构成细胞骨架、赋予细胞结构完整性的脚手架。Tsang博士称:“通常情况下,细胞骨架看起来像一系列连接的六边形。如果细胞失去其结构,它就失去了发挥功能的能力。”
研究人员还发现,MFRP与另外一个基因CTRP5共同发挥作用,正常的肌动蛋白调控需要这两个基因之间的平衡。
 
在下一阶段的研究中,CUMC研究小组使用腺相关病毒(AAVs),将MFRP的正常拷贝,引入iPS来源的视网膜细胞中,成功地恢复了细胞的功能。研究人员还利用基因疗法来“拯救” MFRP基因突变导致的RP小鼠。根据Tsang博士介绍,小鼠的视觉功能得到了长期改善,光感受器数目得以恢复。
 
本文共同作者、CUMC发育细胞生物学助理教授Dieter Egli博士指出:“这项研究提供了体内和体外证据表明,MFRP基因突变引起的视力丧失,可以用AAV基因疗法进行治疗。”
 
Tsang博士认为这种方法也可以用于研究其他RP形式。他说:“通过基因组测序研究,我们发现有数百个遗传拼写错误与RP相关。但直到现在,我们很难能查明是否这些拼写错误真正会引起RP。原则上,iPS细胞可以帮助我们确定这些基因是否真的会引起RP,了解它们的功能,并最终制定个性化的治疗方法。”
