ACC 2015:胆固醇理论的又一论证 OSLER研究延长期随访结果

胆固醇理论认为,胆固醇是动脉粥样硬化斑块的主要成分,没有胆固醇就没有动脉粥样硬化性心血管疾病。20余年来先后结束的大量临床研究证实,对于心血管患者及其高危人群,应用他汀类药物的主要作用机制仍存在争议。多数学者认为,他汀主要通过降低胆固醇水平发挥有益效应,但也有学者认为此类药物的多效性(抗炎、抗氧化应激、保护血管内皮功能等)起着关键作用。半个多世纪以来,多项临床试验均未证实非他汀类药物(贝特类、烟酸、鱼油制剂等)可以改善患者预后,因此有专家认为通过降脂改善预后可能是他汀类药物所特有的。

不久前揭晓的IMPROVE-IT研究表明,与他汀单药治疗相比,联合应用他汀和依折麦布可进一步降低胆固醇水平,并更为有效的减少了急性冠脉综合征患者不良心血管事件的发生。这项研究结果有力证明,应用非他汀类药物降低胆固醇水平同样可以使患者获益,从而论证了“降胆固醇是硬道理”的理念,为胆固醇理论提供了新的临床研究证据。

2015年ACC年会期间揭晓了OSLER研究受试者延长期随访结果,进一步论证了胆固醇理论的普适性。该研究采用开放标签设计,共纳入参加2期与3期Evolocumab(PCSK 9单克隆抗体)临床试验的4465例患者,均予以标准化常规治疗,在此基础上按照2:1的比例随机纳入Evolocumab(2976)或安慰剂(1489例)治疗组。Evolocumab组患者接受每2周一次Evolocumab 140mg皮下注射或420mg每月1次皮下注射。平均随访11.1个月。主要终点为由死亡、心肌梗死、不稳定性心绞痛、冠状动脉血运重建、卒中、短暂脑缺血发作和慢性心力衰竭住院所组成的复合终点。随访结束时,Evolocumab较安慰剂组受试者LDL-C降至<70 mg/dl(差幅约61%)(P<0.001),LDL-C降至<70mg/dl者的比例分别为73.6%与3.8%(P<0.0001),两组主要终点事件发生率分别为0.14%与0.41%(P=0.33)。Evolocumab组患者冠动脉事件与脑血管事件发生率均较安慰剂组呈现下降趋势,但未达到统计学显著性差异。两组间不良事件发生率无异常。本研究结果进一步证实了Evolocumab的降胆固醇效果及其安全性,并且继ODYSSEY Long Term研究之后,再次证实PCSK 9抑制剂可以显著降低临床终点事件发生率。

OSLER研究的延长期随访结果令人振奋。本研究的意义不仅Evolocumab以及其他PCSK 9抑制剂的临床应用展示了良好的前景,更为重要的是再一次论证了胆固醇理论。结合关于胆固醇吸收抑制剂依折麦布以及PCSK 9抑制剂Evolocumab的现有研究结论,我们可以认为通过降低胆固醇水平产生临床获益并非仅限于他汀类药物。只要安全而显著的降低胆固醇水平,就可以有效减少动脉粥样硬化性心血管事件的发生。当然,作为一项临床终点试验,OSLER试验的延长期研究尚存在一些局限性,我们仍需要更大规模,设计严谨的随机化对照试验进一步探讨Evolocumab以及PCSK 9抑制剂的确切疗效,特别是对心血管及其高危人群远期心血管硬终点事件的影响。
