Alzheimers Dementia:整合8个国家,1万人大数据:多锻炼可降低痴呆风险

每周进行3.1至6.0小时的体力活动和消耗9.1至18.0/MET-小时的能量可能会降低痴呆风险。

痴呆症是造成老年人残疾和依赖的主要原因之一,因为它影响到记忆、思维、行为和进行日常活动的能力。目前,全世界有超过5千万人患有痴呆症,估计到2050年这个数字将接近1.52亿。除了开发有效的药物,一些可改变的风险因素可以作为潜在的手段来减轻人口老龄化带来的日益增长的疾病负担。

身体活动被定义为由骨骼肌肉产生的任何需要能量消耗的身体运动。根据柳叶刀委员会关于痴呆症预防、干预和护理的2020年报告,身体不运动,加上其他11个可改变的风险因素,占全世界痴呆症的40%,因此,理论上可以预防或延缓。世界卫生组织(WHO)的指南和柳叶刀委员会2020年报告都建议老年人保持身体活动以预防痴呆症。然而,并没有推荐的体力活动剂量。

了解体育活动与痴呆症之间的剂量反应关系,对于干预研究的设计和体育活动循证指南的制定都是至关重要的。以前的一些研究已经研究了晚年体育活动和痴呆症风险之间的潜在剂量反应关系。然而,许多研究并没有为计算量(持续时间、频率和强度的乘积)而收集有关体育活动的持续时间、频率和强度的详细信息,而只是测量体育活动的频率或持续时间。在少数计算数量的研究中,对身体活动的分类有很大的不同,结果也不一致。这使我们很难在以前的研究中比较基于异质性评估和不同的身体活动分类的结果。因此,晚年体育活动和痴呆症的剂量反应曲线的确切形状尚不清楚。

国际联盟的记忆队列研究(COSMIC)结合了基于人口的纵向队列研究的数据,以确定痴呆症和认知能力下降的共同风险和保护因素。这个联盟为我们提供了一个机会,采用统一的方法来计算和分类多个队列的体力活动。


藉此,复旦大学的Ding Ding等人,为了研究晚年体力活动与发生痴呆症风险之间的剂量反应关系,根据COSMIC的10个队列进行了汇总分析。

他们协调了来自8个国家10个队列的11988名参与者的纵向数据,以研究晚年体育活动与老年人痴呆症发病之间的剂量反应关系。

他们发现:以没有体育活动为参考,痴呆风险随着体育活动时间的增加而降低,最高可达3.1至6.0小时/周(0.1至3.0小时/周的危险比[HR]0.88,95%置信区间[CI]0.67至1.15;3.1至6.0小时/周的HR 0.68,95%CI 0.52至0.89),但随着时间的增加而趋于平稳。

对于体力活动量,观察到类似的剂量-反应曲线模式,拐点是9.1至18.0代谢当量值(MET)-小时/周(0.1至9.0 MET-小时/周的HR 0.92,95% CI 0.70至1.22;9.1至18.0 MET-小时/周的HR 0.70,95% CI 0.53至0.93)。

这项研究的重要意义在于发现了:每周进行3.1至6.0小时的体力活动和消耗9.1至18.0/MET-小时的能量可能会降低痴呆风险。
